The neck is a masterpiece of biomechanical engineering. This delicate column of vertebrae not only supports the weight of the head but also provides the greatest range of motion for seeing and turning. What is cervical disc problem?
Our cervical spine is composed of seven vertebrae (C1 to C7). - Cervical Myelopathy: This is the most serious and dangerous complication of a cervical disc problem. While radiculopathy affects individual nerve roots, myelopathy is a more serious condition that occurs when a herniated disc compresses the spinal cord itself. The symptoms of myelopathy are not just pain; they are functional impairments:
- Loss of dexterity In the hands (e.g., difficulty writing, buttoning clothes, or picking up small objects).
- Difficulty walking and loss of balance.
- Sensation of weakness and heaviness in the legs.

Cervical Disc Diagnosis

- Physical Examination: The doctor checks muscle strength in the hands, reflexes, and sensory perception in the arms and fingers. They may also perform specific maneuvers (like applying downward pressure on the head or rotating it) to try and reproduce the radiating pain and pinpoint the exact location of the affected nerve.
- MRI (Magnetic Resonance Imaging): MRI is the gold standard for diagnosing disc problems. It shows soft tissues, including the disc itself, nerve roots, and most importantly, the condition of the spinal cord in complete detail.
- Nerve Conduction Study / Electromyography (NCS/EMG): This test is particularly useful for confirming nerve damage and evaluating signal dysfunction when the diagnosis is unclear.
Cervical Disc Treatment
1. Non-Surgical Treatments:
- Activity Modification: The first step is to avoid movements that trigger the pain. Using a soft cervical collar for a few days can help relax the muscles.
- Medication: Anti-inflammatory drugs significantly help reduce swelling and inflammation around the nerve root.
- Physical Therapy: The physiotherapist focuses on correcting posture, gentle stretching exercises, and strengthening the deep neck muscles to restore stability to the neck.
- Injections: If the radiating arm pain Is severe and unresponsive to medication, the doctor may suggest a Cervical Epidural Steroid Injection. In this procedure, a powerful anti-inflammatory drug is injected directly Into the area around the inflamed nerve to quickly control the pain.
- Surgery: Surgery is only considered for specific cases:
- Presence of myelopathy symptoms (pressure on the spinal cord).
- Progressive neurological weakness (e.g., the patient’s hand becomes progressively weaker).
- Severe, debilitating radiculopathy pain that shows no improvement after 6 to 8 weeks of conservative treatment.
The most common surgeries are Anterior Cervical Discectomy and Fusion (ACDF) or Artificial Disc Replacement, both aimed at relieving pressure on the nerve or spinal cord.
